The Patient Assessment Standards Coordinator (PASC) ensures accurate extraction, collection, coding, entry, and timely transmission of patient assessment data from clinical documentation, particularly for Medicare patients and the IRF-PAI. The coordinator supports compliance with CMS and Joint Commission standards, educates and trains staff on accurate and defensible documentation, and serves as the primary resource for problem-solving related to Quality Indicator (QI) coding and IRF-PAI completion. The role uses discretion and independent judgment and partners with clinical and operational leaders to ensure the required documentation is complete and patient records support accurate assessment data. The PASC reviews and interprets patient information, participates in team conferences, collaborates with colleagues on comorbidities and CMG/RIC categories, and audits charts and staff scoring for accuracy. The coordinator tracks assessment and documentation measures, maintains audit records and QI certification, prepares PAS audits and reports, analyzes data for improvement opportunities, and works with the leadership team to address them. This is an in-person, in-house hospital position and is not remote.
