About The Position

The Patient Assessment Standards Coordinator (PASC) ensures accurate extraction, collection, entry, and timely transmission of patient assessment data for Medicare patients using the IRF-PAI. The role supports compliance with CMS and Joint Commission standards, promotes accurate and defensible clinical documentation, and serves as a resource and educator for staff on Quality Indicator (QI) coding and IRF-PAI completion. The coordinator uses discretion and independent judgment, partners with clinical and administrative leaders, reviews patient records and data, and identifies opportunities to improve assessment and data-collection processes. This position is performed in person at the hospital and is not remote.

Requirements

• Licensed or certified clinician as defined by state practice standards; listed disciplines include RN, LPN, PT, PTA, OT, OTA/COTA, SLP, Recreational Therapist, Respiratory Therapist, or case manager/social worker.
• QI Credentialed.
• Ability to perform assessments.
• At least 1 year of experience in a healthcare setting.
• Attend all required EHC IRF-PAI trainings.
• Obtain UDS IRF PPS Certification after 2 years in the PASC role and before the third-year anniversary; pass the certification exam and retest every 2 years to maintain certification.
• Work in person at the hospital; the position is not remote.

Responsibilities

• Ensure compliance with regulations for IRF-PAI completion, including admission and discharge QI coding.
• Accurately extract, review, interpret, collect, and enter patient data from clinical documentation for IRF-PAI completion.
• Coordinate accurate and timely data submission for Medicare patients and transmit IRF-PAI data to CMS/UDS within required time frames.
• Educate and train staff and medical staff on the IRF-PAI manual, QI coding, and accurate, defensible documentation.
• Act as a primary resource for immediate problem-solving related to QI coding and IRF-PAI completion.
• Improve QI coding and other IRF-PAI data-collection processes and help staff understand how documentation and data extraction affect hospital performance.
• Ensure confidentiality of Protected Health Information.
• Review charts and observe staff scoring to ensure accurate QI coding.
• Participate in Team Conference, sharing and receiving information about patient condition, functional performance, and goals.
• Collaborate with the team to identify potential comorbidities and accurate CMG/RIC categories.
• Ensure discharged patient records contain required elements and complete chart audits for timely UDS export and CMS transmission.
• Maintain QI certification and a tracking system for admission reference dates and IRF-PAI completion dates.
• Track admission and discharge changes, Self-Care and Mobility items, risk-adjusted outcomes, CMI, CMGs, IRF-PAI item completion, and CMS-13 compliance.
• Analyze data to identify improvement opportunities and work with the leadership team to address them.
• Prepare and ensure completion of PAS audits and reports; review the Transmission Validation Report and maintain audit records.
• Attend required EHC and UDS training and certification programs and maintain PPS certification through retesting every two years.
• Maintain current licensure and certifications, when applicable.
• Report questionable situations, concerns, complaints, or harassment immediately.
• Organize, plan, and manage time effectively; complete assignments and mandatory training by required deadlines.
• Adhere to CMS and Joint Commission standards.
