About The Position

This position is responsible for supporting residents at Pathways Village Apartments to identify goals, needs, and resources for long term household stabilization. Pathways Village is a 40-unit permanent supported housing facility for people who have experienced chronic homelessness. Residents receive supportive services such as resource referral, crisis intervention, resume building, completing paperwork, and more from two Hilltop Navigators who are officed at the apartment building. Requirements

  • Associate Degree in Human Services or related field or four years of related experience in the Human Services field.
  • Two years of lived experience (i.e., housing insecurity, behavioral health, food insecurity or recovery from addiction) is required.
  • Valid Colorado Driver’s License

Nice To Haves

  • CPR Certificate/First Aid Certificate (preferred)

Responsibilities

  • Become trained and/or certified in assigned Hilltop Program, curriculum or practice, and conduct duties in accordance with fidelity standards, grant requirements and program guidelines.
  • Complete certifications and trainings required for position, which may include, but not be limited to Homeless Management Information System (HMIS), Credentials of the Colorado Standards for Peer and Family Specialist, Credentials of the Family Resource Center (FRC), Parents as Teachers (PaT), or other required training, as applicable to assigned role.
  • Utilizing both a person-centered and trauma-informed care approach, assist participants in creating/identifying and reaching goals aimed at long term stabilization for the household.
  • Administer all required assessments and screenings with participants.
  • Maintain new and current participant data using program-established documentation processes, including creating and maintaining physical and electronic participant files.
  • Develop understanding of other Hilltop Programs, external services and community partners to refer participants to when additional assistance is needed.
  • Collaborate with relevant agencies to assist participants in meeting identified goals and maintaining long-term household stability.
  • Develop and maintain up-to-date knowledge and information on programs and community resources.
  • Lead and facilitate engaging educational and social workshops for residents, fostering community connection and personal development.
  • Assist with conflict resolution between participants, property management, GJ Housing Authority and/or other housing providers as needed.
