The PAT Parent Educator provides personal home visits, child development screenings, and family support services to eligible prenatal mothers and families with young children. Operating under the Evidence-Based Parent as Teachers (PAT) model funded by Department of Health (DOH) – Maternal, Infant and Early Childhood Home Visiting (MIECHV) program, this role focuses on strengthening protective factors, promoting positive parenting, and ensuring school readiness. This position requires strict adherence to model fidelity and precise data tracking for federal performance measures. The role involves conducting regular, structured 60-minute home visits with each assigned family to deliver individualized, developmentally appropriate educational activities using the Parent as Teachers (PAT) curriculum model. It also includes planning and facilitating monthly parent group connections events to provide group learning experiences that promote social-emotional and cognitive development, managing an active caseload of 20-25 families, and conducting ongoing Home Safety Checklist assessments during home visits to identify potential risks and support safe home environments. The position requires logging home visit, family goals, referrals, and follow-up data within required local (DOH) and PAT database systems within 3 to 5 business days of interactions, and tracking federal MIECHV performance benchmarks, demographic data, and participant attrition metrics. Developmental and behavioral screenings must be administered within 90 days of enrollment and according to the child level screening schedule compliance and timelines. The role also includes attending Continuous Quality Improvement (CQI) data review meetings to analyze program performance measures and actively recruiting eligible prenatal families and families with children 0-3 years old (prioritizing early enrollment per MIECHV guidelines) through community events, clinics, canvassing, etc.
