The PASRR (Preadmission Screening and Resident Review) Diversion Coordinator is responsible for provision of education, quality oversight, and compliance reporting for persons with intellectual developmental disabilities (IDD) diverting from admission to a nursing facility. The PASRR Diversion Coordinator supervises a team of Habilitation Coordinators who perform case management for persons with IDD or IDD and mental illness (MI) residing in a nursing facility and for persons with IDD who diverted or transitioned from institutional care. The duties of the position include, but are not limited to the following: Supervise and assist with completion of PASRR Evaluations for persons admitted to nursing facilities who screen positive for IDD or for IDD and mental illness (MI) Manage Habilitation Coordination processes for persons with IDD or MI residing in nursing facilities, including Community Living Options, transition planning, and coordination of specialized habilitation services Manage Enhanced Community Coordination processes for persons with IDD diverting or transitioning from institutional care, including diversion planning and transition monitoring Complete monthly reporting to the state on performance measures Adhere to and promote of the Texana Compliance Policy Work hours: M-F, 8-5, with flexibility to meet PASRR timelines
