About The Position
Abby Care is seeking a dedicated and experienced professional to join us as PASA Case Manager. This role will report into the PASA Program Lead – Colorado. This is a Full-Time Hybrid position based in Denver, CO. Abby Care is building the leading AI-native platform for family-led care. America is facing a growing care crisis. Requirements
- 2-3 years of direct experience as a case manager or similar role specifically within Colorado's PASA program (CES/SLS Waivers).
- Familiarity with PASA services such as Community Connector, Homemaker Enhanced, Respite, or Supported Community Connector.
- Strong organizational skills with proficiency in Google Workspace, Salesforce, or equivalent tools for documentation and reporting.
- Excellent communication and empathy, with the ability to support diverse families in a person-centered approach.
- Ability to work independently in a fast-paced, hybrid environment while thriving in ambiguity.
- Located in the Denver metro area; willing to travel across Colorado as needed.
