About The Position

This position provides in-home support to families, that promotes a positive parent-child relationship and interaction and provides case management/linkage to community resources to parents/caregivers. To support and advocate for the family to receive their entitlements and benefits, housing connects, etc.

Requirements

  • Ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful caring manner.
  • Demonstrates ability to organize and coordinate work within schedule constraints and handle emergent requirements in a timely manner.
  • Ability to communicate effectively with supervisors, co-workers, and other community partners
  • Ability to conduct interviews with families to obtain and provide information needed to carry out case management responsibilities
  • Knowledge and ability to use critical thinking and analytical skills used to formulate with the family person achievable plans for implementation
  • Ability to perform the essential functions of the job with or without a reasonable accommodation.
  • Collaboration and facilitation skills that to work with the individual, families and services providers.
  • Demonstrates cultural sensitivity and competence in work with families from various ethnic and cultural groups, social economic status and lifestyles.
  • HS Diploma or GED required
  • Basic computer literacy preferred.
  • Successful completion of an Administration for Children’s Services-funded prevention services program or similar program or Life/lived experience with the child welfare system required

Responsibilities

  • Conduct field visits and escort families to various service providers; medical appointments, Public Assistance centers, Medicaid offices and housing services.
  • Conduct home visit in compliance with ACS FTR requirements in which the needs of the family and child safety are assessed.
  • Advocate and assist families with connecting to community-based resources/ organizations and making referrals to facilitate the services needed to the families.
  • Provide support services to families receiving services; which includes supportive casework focusing on services goals, child welfare laws, improving family relations ships, coping skills and parenting etc.
  • Obtain collateral documents from medical, treatment and academic programs.
  • Works with parents and provide education on improving parenting skills and home management skills.
  • Participates in monthly Unit and Clinical Diagnostic Team Meetings.
  • Completes and documents case work activities in tracking logs/excel sheets, case record PROMIS, CONNECTIONS and all other systems on a timely manner.
  • Attends all required trainings to enhance skills working with high risk population.
  • Conducts case record/chart reviews on a monthly basis.
  • Schedules and attends meetings and conferences as needed.
  • Conduct community outreach and relationships with external providers.
  • Conduct monthly chart reviews of at least 15 case records per month.
  • Complete sections of the FASP assessment as required by ACS and OCFS
  • Meets required weekly face-to-face contacts with all parents and children assigned.
  • Participates in School visits, IEP meetings.
  • Monitor and obtain information on the child/ren school placement and educational performance.
  • Monitor and obtain information on the family’s medical needs, including compliance with periodic physicals and immunizations.
  • Working with individuals, family and friend support networks, and health care and mental health professionals to put services in place.
