About The Position

Indigo Sun Palliative Care is seeking a compassionate and experienced Palliative Registered Nurse (RN) to join their growing team. As a Palliative RN, you will provide skilled nursing care, comprehensive patient assessments, symptom management, and care coordination for patients living with chronic or life-limiting illnesses. Unlike hospice care, our focus is on improving quality of life while patients continue receiving curative or life-prolonging treatments when appropriate. This position is ideal for a nurse who enjoys building meaningful relationships, educating patients and families, and collaborating with physicians and interdisciplinary team members to deliver exceptional home-based care.

Requirements

  • Graduate of an accredited school of nursing.
  • Current and unrestricted Registered Nurse (RN) license in the State of New Mexico.
  • Minimum of one year of nursing experience.
  • Current driver's license, reliable transportation, and proof of automobile insurance.
  • Strong assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.

Nice To Haves

  • Experience in palliative care, home health, hospice, or case management.
  • Experience with electronic health record (EHR) systems.
  • Knowledge of chronic disease management and community resources.

Responsibilities

  • Perform comprehensive in-home nursing assessments and ongoing patient evaluations.
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
  • Monitor symptoms, provide clinical interventions, and communicate changes in patient condition to providers.
  • Educate patients and caregivers on disease processes, medications, symptom management, and available community resources.
  • Coordinate care with physicians, specialists, therapists, social workers, and other healthcare professionals.
  • Advocate for patient goals and preferences while promoting comfort, independence, and quality of life.
  • Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
  • Participate in interdisciplinary team meetings and case conferences.
  • Participate in the on-call rotation as scheduled.
