About The Position

Our mission is to improve quality of life by providing expert symptom management, education, and care coordination in the comfort of our patients' homes. We partner with patients, families, and healthcare providers to help individuals navigate complex medical conditions while maintaining independence, dignity, and peace of mind. We are seeking a compassionate and experienced Palliative Registered Nurse (RN) to join our growing team. As a Palliative RN, you will provide skilled nursing care, comprehensive patient assessments, symptom management, and care coordination for patients living with chronic or life-limiting illnesses. Unlike hospice care, our focus is on improving quality of life while patients continue receiving curative or life-prolonging treatments when appropriate. This position is ideal for a nurse who enjoys building meaningful relationships, educating patients and families, and collaborating with physicians and interdisciplinary team members to deliver exceptional home-based care.

Requirements

  • Graduate of an accredited school of nursing.
  • Current and unrestricted Registered Nurse (RN) license in the State of New Mexico.
  • Minimum of one year of nursing experience.
  • Current driver's license, reliable transportation, and proof of automobile insurance.
  • Strong assessment, communication, and critical thinking skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.

Nice To Haves

  • Experience in palliative care, home health, hospice, or case management.
  • Experience with electronic health record (EHR) systems.
  • Knowledge of chronic disease management and community resources.

Responsibilities

  • Perform comprehensive in-home nursing assessments and ongoing patient evaluations.
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
  • Monitor symptoms, provide clinical interventions, and communicate changes in patient condition to providers.
  • Educate patients and caregivers on disease processes, medications, symptom management, and available community resources.
  • Coordinate care with physicians, specialists, therapists, social workers, and other healthcare professionals.
  • Advocate for patient goals and preferences while promoting comfort, independence, and quality of life.
  • Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
  • Participate in interdisciplinary team meetings and case conferences.
  • Participate in the on-call rotation as scheduled.
