About The Position

Salus Hospice is seeking a compassionate and skilled Palliative/Hospice Nurse Practitioner to serve patients in the Orange County region. The position works closely with a physician and an interdisciplinary team to provide patient-centered hospice care, help ensure patients meet face-to-face visit requirements under applicable regulations, and support patients and families. The role involves travel across a large geographic area to visit patients and pays $72–$85 per hour. It is a per diem position with a flexible schedule.

Requirements

At least 2 years of recent experience in hospice or palliative care.
Expertise in providing care to patients with end-stage disease.
Active Nurse Practitioner certification in California.
California state licensure to practice as a Nurse Practitioner.
Willingness to travel within a large geographic area to visit patients.
Experience delivering high-quality, compassionate hospice care.
Deep understanding of end-of-life care and regulations.
Ability to follow Standardized Procedures based on guidelines established by the Board of Registered Nursing Practice Act.

Nice To Haves

Master’s degree in Nursing preferred.

Responsibilities

Conduct face-to-face patient visits to evaluate continued hospice eligibility in line with new regulations.
Collaborate with physicians and the interdisciplinary team, share clinical findings, and adjust care plans.
Perform bedside evaluations and create comprehensive care plans tailored to patient needs.
Provide direct hospice care and support patients and families.
Document findings in accordance with CMS guidelines and ensure accurate compliance.
Consult with supervising physicians and collaborate with healthcare providers within and outside Salus Hospice.
