About The Position
Paradigm Health is hiring a full-time Palliative Nurse Practitioner to provide compassionate, high-quality care to patients and families across a local Indiana territory. The role is scheduled Monday through Friday for 40 hours per week and includes participation in the provider on-call rotation. The position provides medical support and care coordination for palliative care patients and their families.
Requirements
- Be licensed as a Nurse Practitioner in the State of Indiana.
- Provide proof of current classroom CPR.
- Have a current driver’s license.
- Have reliable transportation and insurance.
Nice To Haves
- One year of Nurse Practitioner experience is strongly preferred.
Responsibilities
- Provide medical support and coordination to palliative care patients and family members.
- Complete history and physical examinations by evaluating patients’ medical conditions and health histories.
- Manage acute and chronic health conditions and terminal illnesses by developing individualized plans of care, prescribing medications and treatments, and obtaining consultations or making referrals.
- Assess and reassess pain, use appropriate pain management techniques, and educate patients and families about pain management.
- Sign orders and medication prescriptions for palliative patients efficiently.
- Participate in the provider on-call rotation.
