About The Position
Responsible for performing and documenting psychosocial assessments, coordinates educational and outreach services relevant to the social services, and provides liaison function between system-wide outpatient clinics.
Requirements
- Master’s degree in Social Work from an accredited institution
- Licensed Clinical Social Worker (LCSW) certification required
- Strong understanding of end-of-life care and ethical issues in Palliative Care.
- Minimum of 2 years’ experience in a healthcare setting
Nice To Haves
- Experience in Palliative Care or hospice setting preferred
- Familiarity with Epic, ICD-10, and CPT coding preferred.
