About The Position

This position serves as a liaison for transitional care planning between patients, caregivers, and palliative care/clinical care teams. The Social Worker will assist clients in navigating healthcare systems and identifying/linking them with resources. They will attend relevant administrative and clinical meetings, participate in interdisciplinary team meetings, care planning, and ethics consultations. The role involves independent psychosocial assessment, direct clinical counseling, and providing grief, loss, and bereavement support. Psychosocial education will be provided to patients and family caregivers regarding coping strategies, palliative care philosophy, and non-pharmacological symptom management. The Social Worker will utilize crisis intervention and problem-solving skills, advocate on behalf of patients and families, and facilitate advance care planning. They will also facilitate or support psychoeducation support groups and provide inservices or education workshops to promote the palliative care philosophy. Maintaining effective documentation of social work activities is also a key responsibility. This position is security-sensitive and subject to Texas Education Code 51.215.

Requirements

• Master's Degree in Social Work from an institution certified by the Council of Social Work Education
• Applicable state license required; Upon Hire
• (LCSW) LIC CLINCAL SOCIAL WORKER required or must obtain within 1 year of hire.

Nice To Haves

• 3 years of experience providing direct clinical counseling services preferred.
• certification in Advanced Hospice and Palliative Care preferred. Upon Hire

Responsibilities

• Serves as a liaison for transitional care planning between patient, caregivers and palliative care/clinical care teams.
• Assists clients to navigate healthcare systems; identifies and links clients with resources.
• Attends administrative and clinical meetings relevant to operations of palliative care program; distributes relevant information to departmental staff.
• Participates in interdisciplinary team meetings, care planning and ethics consultations.
• Works with the multidisciplinary team to identify and develop disposition plans.
• Responsible for independent psychosocial assessment and direct clinical counseling.
• Provides counseling and psychotherapy for individuals, couples and families.
• Provides grief, loss and bereavement support to patient and families.
• Provides psychosocial education to patients and family caregivers about coping strategies, hospice and palliative care philosophy and non-pharmacological symptom management strategies.
• Utilizes crisis intervention and problem solving skills to facilitate and enhance patient/family's ability to meet acute and ongoing medical and psychosocial needs.
• Advocates on behalf of the patient and family.
• Facilitates advance care planning.
• Facilitates and/or supports existing psychoeducation support groups.
• Provides inservices or education workshops to other service providers and/or organizations to promote the palliative care philosophy and program.
• Maintains effective documents of social work activities.
• Performs other duties as assigned.
