PRN Palliative Care RN

Four Seasons, The Care You Trust•Franklin, NC

About The Position

The PRN Palliative Care Primary Nurse (RN) supports the Community-Based Palliative Care team by providing clinical and operational coverage during planned and unplanned absences of regularly assigned nurses. This as-needed role has no regularly scheduled caseload and does not guarantee a set number of hours. The RN provides telephonic and, when assigned and appropriate, face-to-face support to patients, families, and the clinical team; assesses patient needs and changes in condition; communicates findings to appropriate providers; and implements authorized orders within the scope of nursing practice. The nurse may also assist with referral reviews, intake screening, medication coordination, prior authorizations, durable medical equipment coordination, and other routine RN workflows to maintain continuity of care and support quality and team operations. The position works with patients who have chronic illnesses and their families, coordinating care and helping clinicians work efficiently. The RN collaborates with Palliative Care leadership, providers, and RN colleagues to prioritize work, communicate outstanding needs, and complete assigned tasks within established timeframes. Assignments and hours depend on coverage needs and mutual availability.
