Four Seasons, The Care You Trust•Franklin, NC

About The Position

The PRN Palliative Care Primary Nurse supports the Community-Based Palliative Care team by providing clinical and operational coverage during planned and unplanned absences of regularly assigned Palliative Care RNs. This is an as-needed position with no dedicated regional caseload and no guaranteed number of hours. Assignments and hours depend on departmental coverage needs and mutual availability. The RN provides telephonic and, when assigned and appropriate, face-to-face support to patients, families, and the clinical team. The nurse assesses patient needs and changes in condition, communicates findings to appropriate providers, and implements authorized orders within the scope of nursing practice. Coverage may also include referral reviews, intake screening, medication-related tasks, prior authorizations, durable medical equipment coordination, telephonic follow-up, and other routine RN workflows that support continuity of care and clinical operations. The RN collaborates with Palliative Care leadership, providers, and RN colleagues to prioritize work, communicate outstanding needs, and complete assigned tasks within established timeframes. The role requires compliance with applicable nursing laws and regulations, company policies, safety and infection-control practices, and the organization's mission and values.
