About The Position

This is a part-time (20 hours per week) day shift position for a Palliative Care Nurse Specialist at Nantucket Cottage Hospital. The role involves providing direct patient care and clinical management for patients and their families. The Palliative Care Nurse works collaboratively with the Nurse Practitioner Director of PASCON to deliver comprehensive palliative care. This includes coordinating with physicians, mid-level providers, nurses, social workers, and external agency staff to ensure team-based, coordinated, and comprehensive care across various settings such as hospitals, homes, and nursing homes, facilitating seamless transitions for patients.

Requirements

  • Bachelor's Degree in Nursing required.
  • MA RN license required.
  • Nursing experience of 2-3 years required.
  • Strong clinical skills.
  • Effective collaboration skill ability with all levels.
  • Ability to rapidly make accurate decisions, analyze, and interpret information.
  • Willingness to be flexible with weekday schedule/patient assignment.
  • Strong communication and interpersonal skills with demonstrated ability to teach and develop others.
  • Strong EMR/computer skills.
  • Willing to learn/become trained in the field of Hospice and Palliative Care.

Nice To Haves

  • Master's Degree in Nursing preferred.
  • Hospice and palliative nursing care experience of 2-3 years preferred.

Responsibilities

  • Organize, develop, plan, implement, and evaluate Palliative & Supportive Care Program goals, objectives, policies, and procedures in accordance with hospital policy and appropriate standards, in collaboration with the Palliative Care Department Nurse Practitioner Manager.
  • Provide high-quality direct patient care, including education and counseling, to departmental patients in any setting – home, hospital, office, or nursing home.
  • Perform and record the patient history and physical exam, discussing findings with the NP Manager and physician as needed.
  • Follow and manage patients in collaboration with the NP manager and physician, enhancing continuity of care.
  • Perform, oversee, and assure completion of specialized procedures mutually agreed upon with the collaborating physician.
