Requirements

• MSN or higher-level Adult Nurse Practitioner required.
• Licensure (or eligibility for licensure) for advanced nursing practice in both KY and IN, including DEA and controlled substance permits, required.
• Fulfill requirements of on-call duty policies.
• Must have a valid driver’s license in accordance to policy.

Nice To Haves

• Minimum of one year experience in full time advance nursing practice preferred.
• Primary care, palliative care, hospital-based, or hospice experience preferred.
• Use of technology-based tools, including electronic medical record, strongly preferred.

Responsibilities

• Provides direct patient care, including making medically-necessary palliative care visits (billable) and hospice face-to-face recertification (FTF) visits (non-billable).
• Will also rotate to cover general Hosparus Health -wide weekday and weekend call for orders and prescriptions.
• Supports and coordinates care with the Interdisciplinary Group (IDG).
• Participates in education, outreach and leadership activities for the community location.
• May be assigned additional patient care or administrative duties at the discretion of the Senior Nurse Practitioner or VP/Chief Medical Officer.
