About The Position

Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner on a Per Diem (PRN) basis to join our growing healthcare team in Mentor, OH. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care. This role offers a flexible Monday through Friday schedule with no nights, weekends, or holidays, providing an opportunity for experienced NPs seeking a flexible, well-supported role.

Requirements

Active license as a Nurse Practitioner (NP)
Minimum 2 years of post-certification experience
Proficient with EHRs and clinical technology
Strong communicator with a collaborative mindset

Nice To Haves

Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred

Responsibilities

Deliver person-centered care using evidence-based practices
Collaborate with interdisciplinary facility teams
Conduct assessments, develop treatment plans, and manage follow-up care
Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
Educate patients, families, and staff
Complete timely documentation (within 24 hours)
Compliance with facility, state and federal regulations/protocols.

Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)
