Palliative Care Nurse Practitioner Per Diem (PRN) - Marlborough, Mass
About The Position
Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner on a Per Diem (PRN) basis to join our growing healthcare team in Marlborough, Mass. Impact Health PC is a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. We specialize in Medical, Psychiatric, Wound Care and Palliative Care. We are looking for an experienced NP seeking a flexible, well-supported role to thrive in a collaborative environment that values expertise and independence, with the freedom of a flexible Monday through Friday schedule with no nights, weekends, or holidays.
Requirements
- Active license as a Nurse Practitioner (NP)
- Minimum 2 years of post-certification experience
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
Nice To Haves
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
Responsibilities
- Deliver person-centered care using evidence-based practices
- Collaborate with interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Compliance with facility, state and federal regulations/protocols.
