About The Position

This role is for a social worker who is ready to take on more responsibility, shape a meaningful program, and lead while staying grounded in their work. The Palliative Care Manager will have a voice that matters, ideas that shape the future, and work that directly impacts patients and families during significant life moments. The individual will join a mission-driven team that values courage, compassion, and creativity, helping to launch and grow a brand-new palliative care program from the ground up at Emmanuel Hospice. This position offers a unique opportunity to be a cornerstone in launching Emmanuel Hospice’s new palliative care program, shaping its vision, structure, and long-term impact. As a key leader, the manager will guide the development of innovative care models to support patients facing serious illness and their families. Their expertise will influence the program's growth, evolution, and integration into the Emmanuel Hospice continuum of care. Reporting directly to the CEO, the Palliative Care Manager ensures the program upholds excellence, regulatory compliance, and the mission to support patients and families with serious illness.

Requirements

  • Master's degree in social work required
  • Experience in Electronic Medical Records and Microsoft Office products is required
  • Strong interpersonal, communication, and leadership skills with a demonstrated ability to collaborate effectively across disciplines, and guide teams through periods of growth and change.

Nice To Haves

  • Hospice or palliative care related leadership experience preferred
  • Prior program development experience preferred

Responsibilities

  • Lead all program startup activities, including workflows, policies, and operational design
  • Manage program budget, financial performance
  • Supervise and support palliative care staff in collaboration with the Medical Director
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
  • Support provider credentialing and payer contracting
  • Ensure efficient scheduling, referrals, documentation, and care transitions
  • Drive program growth, customer service, and performance improvement
  • Deliver social work services during start up and as needed
  • Participate in the Manager On-Call Rotation for hospice services
