About The Position

VNA of Ohio is seeking an experienced RN to fill a unique, growth-oriented role as their Palliative Care Clinical Supervisor. This position offers the opportunity to shape a program from its early stages. Initially, the role will function as an RN Case Manager in the field while the palliative program grows. As the program expands, the role will transition into a full clinical supervisor position. This is an opportunity for a clinician with leadership ambitions and a passion for serious illness care to grow within the organization.

Requirements

  • Active RN license (Ohio) + current BLS certification
  • Bachelor's degree in Nursing (BSN) required
  • Minimum 3 years of clinical nursing experience, including experience in palliative care, hospice, or home health
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Familiarity with Medicare and Medicaid regulations
  • Strong clinical assessment, care planning, and documentation skills

Nice To Haves

  • MSN preferred
  • Prior clinical leadership or supervisory experience
  • CHPN certification or willingness to pursue
  • Experience building or growing a clinical program
  • Familiarity with HCHB or similar home health/hospice EMR
  • Strong interpersonal and team development skills

Responsibilities

  • Provide skilled nursing care to palliative care patients in their homes in accordance with the established plan of care.
  • Conduct comprehensive patient assessments and develop individualized, goals-of-care focused care plans.
  • Manage a palliative care caseload, coordinating services across the interdisciplinary team.
  • Educate patients and families on disease management, comfort-focused care, and available resources.
  • Maintain accurate and timely documentation in the EMR within required timeframes.
  • Ensure compliance with Medicare, Medicaid, and agency regulations.
  • Provide clinical oversight and supervision to palliative care field staff.
  • Support onboarding, training, and ongoing development of palliative care clinicians.
  • Monitor caseloads, quality indicators, and patient outcomes across the palliative program.
  • Collaborate with leadership to develop and refine palliative care protocols, workflows, and best practices.
  • Serve as a clinical resource and subject matter expert for the interdisciplinary team.
  • Participate in quality assurance, performance improvement, and accreditation activities.
