CareBridge Health exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through homecare and community based services. The Palliative Care Case Manager is responsible for coordinating referrals, education, and interdisciplinary support for hospice and palliative care services to ensure that members with serious or terminal illnesses receive appropriate care. The role supports accurate member understanding of care goals and facilitates engagement with relevant community resources. This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development.
