About The Position

HopeWest is seeking a Palliative Care Case Manager (RN) for their Grand Junction location. This role focuses on providing compassionate, relationship-based care to patients and families experiencing serious illness through the Palliative Care programs of Transitions, Living with Cancer, and Journeys. The Case Manager will develop and implement individualized patient care plans, assist patients in coordinating care within the healthcare system, focus on comfort and symptom management, discuss goals of care, assist with advance directives, and ensure timely transitions to hospice care when eligible. This is a part-time position, 24 hours per week, offering a weekday schedule with occasional on-call/weekend requirements.

Requirements

Minimum 1–2 years of nursing experience preferred
Experience in home health, palliative care, or acute care strongly preferred
Current Colorado RN license maintained throughout employment
Current CPR certification maintained throughout employment
Valid Colorado driver’s license, reliable transportation, and proof of insurance
Strong communication and interpersonal skills
Ability to navigate emotional or high-stress situations with professionalism and compassion
Experience with EHR systems and Microsoft Office applications

Responsibilities

Oversight and care of patients in the Palliative Care programs of Transitions, Living with Cancer, and Journeys for the Grand Valley.
Development and implementation of individualized patient care plans for patients/families experiencing serious illness.
Assisting patients in coordinating care through the healthcare system while focusing on comfort and symptom management needs for advancing disease.
Discussing goals of care and assisting with advance directives to navigate their care.
Ensuring a timely transition to hospice care when eligible.
