About The Position

This role is ideal for an Advanced Practice Provider who is looking to combine their clinical and program development expertise to grow the Peninsula Palliative Care program in our Hampton, Newport News and Williamsburg offices. In this role, the APP acts as a palliative care consultant to an interdisciplinary team for complex pain and symptom management working in collaboration with providers, social work, nurse navigators and nurses. Requirements

  • Masters of Science in Nursing (MSN) from an accredited program for nursing education and successful completion of a Nurse Practitioner program or successful completion of an accredited Physician Assistant program
  • Previous Palliative Care or Hospice experience required
  • RN State Licensure and current registration with the State Board of Nursing in practicing state, along with current state certification of Nurse Practitioner or current state licensure as a Physician Assistant
  • Current CPR is required
  • Must be willing to travel to various peninsula practice locations
  • Experience and knowledge in the management of the physiological and psychological effects of cancer diagnosis and subsequent treatment

Nice To Haves

  • Post Nurse Practitioner or Physician Assistant hematology/oncology experience is preferred
  • Palliative Care or Hospice experience as a Nurse Practitioner or Physician Assistant is preferred
  • Prefer certification in Palliative or Hospice Palliative Care medicine

Responsibilities

  • Educates and counsels patients and families about the disease process and end of life concerns to ensure a holistic approach to patient care
  • Serves as a liaison to practice and community providers. Maintains good communication and collaborative relationships with referring physicians and other medical staff to ensure patients are receiving necessary care
  • Responsible for managing and mentoring site staff to ensure adequate coverage and high-quality patient care. Conducts team meetings that address, discuss and train on palliative care techniques and recommendations
  • Demonstrates adaptability and flexibility while handling day-to-daywork challenges and adjusting to multiple demands, shifting priorities, ambiguity and rapid change
  • Develop patient schedules to meet the demands of a traveling rotation between Newport News, Hampton and Williamsburg offices
  • Conduct comprehensive assessments of patients with advanced cancer, including physical, emotional and psychosocial aspects
  • Develop and implement individualized care plans in collaboration with oncologists and the palliative care team to ensure management of patient pain, symptoms and side effects of cancer treatments
  • Monitor and adjust pain management strategies to ensure optimal comfort and quality of life for patients
  • Communicate with patients and their families to provide education, support, resources and emotional care
  • Assist in discussing advanced care planning, end-of-life decisions and goals of care
  • Stay current with advances in palliative care, oncology and pain management through ongoing education and professional development opportunities
  • Maintain accurate and thorough medical records, documenting patient assessments, interventions and progress.
  • Ensure compliance with relevant regulations and guideline
