Advanced Practice Provider - Palliative Care
About The Position
Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for an Inpatient and outpatient position to join our team at Franciscan Health Dyer and Franciscan Health Munster to meet the growing demands for Hospice and Palliative Care services. The ideal candidate will have past Palliative Care experience, with a focus on clinical excellence, and patient satisfaction. The candidate will display exceptional interpersonal skills and a desire to develop strong ties with the community and existing medical staff. The provider will need to work collaboratively with a physician to coordinate and deliver the highest quality of patient care. The schedule involves a time split between clinic and inpatient rounding, with rotating weekend call for inpatient only (1:4). The average outpatient volume is 12 patients per week, and the average inpatient volume is 6+ patients per day.
Requirements
- Master's Degree
- Physician Assistant Certified (PA-C) or Nurse Practitioner (NP) or Nationally Recognized Certification
- Basic Life Support Program (BLS) - American Heart Association
- Past Palliative Care experience
- Focus on clinical excellence
- Focus on patient satisfaction
- Exceptional interpersonal skills
- Desire to develop strong ties with the community and existing medical staff
Responsibilities
- Coordinate and deliver the highest quality of patient care in collaboration with a physician.
- Provide care in both inpatient and outpatient settings.
- Manage an average of 12 outpatient patients per week.
- Manage an average of 6+ inpatient patients per day.
- Participate in a rotating weekend call schedule for inpatient care (1:4).
