Mosaic Life Care provides Inpatient Rehabilitation/Post-Acute Care in St. Joseph, MO. Therapy plays a very important role in recovery from a life-altering injury or illness. We offer a hospital-based physical rehabilitation program for the treatment of those who have experienced a debilitating injury or illness. Our Unit is a very fast-paced yet fun environment with endless opportunities to learn and grow!
Requirements
- High School Diploma or GED Equivalent is required.
- Strong organizational and communication skills.
Nice To Haves
- Previous experience working in inpatient rehabilitation is preferred.
Responsibilities
- Provides support to the IRF-PAI Coordinator and/or Program Director/ Designee for oversight and coordination of the timely and accurate completion of the Patient Assessment Instrument for each rehabilitation inpatient.
- Gathers, enters and edits the information contained in the PAI for each patient with review by a clinician to assure accuracy in the scoring of functional status and reflection of the patient’s care needs.
- May assist in the ongoing monitoring of identified benchmarks for the inpatient population.
- Maintains confidentiality of all patient information.
- Supports in coordination of the IRF-PAI completion with the interdisciplinary team.
- Gathers and inputs data, and transmits data to Medicare.
- Assists with education of staff regarding PPS and FIMS/Quality Indicators (QIs).
- Assists with monitoring and management of CMS compliance.
- Other duties as assigned.
