About The Position

The PACE Nurse Manager provides leadership, direction, and operational oversight for nursing services across all PACE sites. This role supervises Clinic RNs, Community RNs, LVNs, Medical Assistants, and Quality Improvement (QI) Coordinators, ensuring scheduling, training, performance management, and quality standards. The Nurse Manager is responsible for oversight and monitoring of home health and hospice services and services provided at skilled nursing facilities (SNF) and residential care facilities for the elderly (RCFE). The PACE Nurse Manager will also lead the implementation of QI activities.

Requirements

  • Graduate of an accredited school of nursing program.
  • Bachelor’s degree or two years' related work experience.
  • Minimum of two years recent acute care experience as an RN.
  • Current California Registered Nurse License.
  • Minimum three years supervisory experience in a healthcare setting.
  • Demonstrated skills in supervision, leadership, and communication and in fostering an effective team environment.
  • Experience managing quality improvement programs and audits.
  • Current BLS and First Aid certification.
  • Must be able to fluently speak, read and write English.
  • Fluency in Chinese (Cantonese) is required.

Nice To Haves

  • Minimum one year of experience working with older adults and their families in a healthcare or community agency setting is preferred.
  • Fluency in other languages is an asset.

Responsibilities

  • Provide direct supervision, training, and evaluation to Clinic RNs, Community RNs, LVNs, Medical Assistants, and QI Coordinators.
  • Ensure effective hiring, training, scheduling, coverage, and staffing across all PACE sites.
  • Delegate responsibilities appropriate to licensure, education, and experience of staff.
  • Develop policies and procedures for all nursing services, clinic operations, and home health services.
  • Work closely with PCPs and other disciplines to develop and implement individualized care plans.
  • Monitor encounter, assessment, and care-plan metrics and identify opportunities to improve overall participant experience.
  • Lead SNF, RCFE, hospice, and home health agency monitoring and coordination to ensure continuity of care.
  • Direct the RN team in coordination between PCPs and hospitals for hospital and SNF stays, ensuring smooth transitions and communication.
  • Lead provision and operational oversight of home health services.
  • Manage the Level of Care (LOC) submission process for prospective participants.
  • Supervise and manage all QI activities and reporting for PACE, ensuring data accuracy and timely submission.
  • Develop and implement QI strategies in collaboration with the Medical Director to meet QI goals.
  • Implement strategies to meet or exceed participant satisfaction benchmarks at national and state levels for nursing services.
  • Prepare for and participate in CMS and DHCS audits, and implement corrective actions as needed.
  • Lead emergency preparedness trainings related to clinic protocols for all PACE staff.
  • Ensure compliance with all PACE regulatory requirements and internal policies.
  • Travel to other PACE sites as needed for supervision and training of staff and to address participant issues.
  • Oversee accurate and cohesive documentation in participant medical records.
  • Oversee the quality of care and safety of PACE participants, both at the PACE center and in the community.
  • Utilize Epic appropriately and efficiently and serve as a content and workflow expert for resolving issues and proposing solutions in Epic.
  • Performs other job duties as required by manager/supervisor.
