About The Position

Sodexo is seeking a compassionate and skilled Registered Dietitian for a full time position with Capital Health LIFE, a Program of All‑Inclusive Care for the Elderly (PACE) that helps seniors remain safe, healthy, and independent in their homes through coordinated medical, social, and supportive services. In this role, you'll manage the nutritional needs of older adults with complex medical and functional challenges, supporting participants across multiple settings—including the LIFE Center, skilled nursing facilities, hospitals, and participant homes—to improve health outcomes, maintain functional ability, and enhance quality of life.

Requirements

  • Registered Dietitian (RD/RDN) credentials
  • Ability to travel to participants’ homes and care facilities (mileage reimbursement provided)
  • Experience with regulatory accreditation and establishing and maintaining standards of care
  • Master's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
  • For those eligible for the registration exam prior to 1/1/24: Bachelor's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
  • Meets eligibility requirements for Registered Dietitian Nutritionist (RD/RDN) by the Commission on Dietetic Registration (CDR) and will become registered within 120 days of hire.
  • Certification/licensure as required by state(s) of practice.
  • 7+ months general supervision
  • 3 years experienced specialized practitioner in a clinical specialty

Nice To Haves

  • Experience in geriatrics, long‑term care, community health, or chronic disease management
  • Excellent communication, leadership and customer service skills
  • In some states and/or facilities the RD/RDN credentials is required at the time of hire
  • In some states and/or facilities state certification/licensure is required at the time of hire

Responsibilities

  • Conduct initial and periodic nutrition assessments in the LIFE Center, participant homes, hospitals, and skilled nursing facilities.
  • Evaluate intake, medical conditions, medications, weight patterns, and cultural dietary preferences.
  • Monitor high‑risk concerns such as malnutrition, weight loss, dysphagia, and wound healing needs.
  • Provide nutrition counseling to participants, caregivers, and staff on special diets, healthy eating, and meal preparation.
  • Work closely with nursing, medical providers, social workers, therapists, and transportation staff to deliver coordinated, participant‑centered care.
