About The Position

The Program of All-Inclusive Care for the Elderly (PACE) is a community-based program that provides coordinated medical and social services to eligible older adults who want to continue living in their own home despite chronic care needs. The Day Center Aide, under direct oversight of the Clinical Leader, is an integral member of the PACE at Hudson Headwaters (PHH) team. The Day Center Aide is responsible for providing support and assistance to Participants during appointments, Day Center activities and collaborates with other members of the PACE Interdisciplinary Team (IDT) to ensure Participants’ care needs are met, and the current plan of care is followed to meet the needs of each Participant.

Requirements

  • High School Diploma or GED
  • A current and valid state-issued Personal Care Assistant (PCA) certificate is required
  • Current BLS certification or willing to complete certification
  • Meet a standardized set of competencies for the specific position description established by PACE before working independently
  • Requires reliable transportation
  • Have 1 year of experience working with a frail or elderly population or be willing to receive appropriate training upon hire
  • Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines
  • Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act
  • Have not been excluded from participating in Medicare and Medicaid programs
  • Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse
  • Ability to work within a care team with initiative, imagination, resourcefulness, and flexibility

Responsibilities

  • Escort Participants to outside appointments and provide assistance when necessary.
  • Assist drivers with loading and unloading Participants from vehicles.
  • Assist the transportation department with Participants’ take-home supplies.
  • Provide and document care in accordance with established policies, guidelines, and applicable regulations.
  • Adhere to PHH safety requirements and Participant care plans regarding use of assistive devices (e.g., gait belts, walker, wheelchair, etc.).
  • Assist Participant Care Associates and other clinical staff with transfers and services including (but not limited to), grooming and oral care.
  • Provide supervision and support to Participants including monitoring and responding to behavioral disturbances such as wandering, agitation, restlessness, and exit seeking while at the Day Center and in the community.
  • Respond appropriately and in a timely manner to personal alarm devices.
  • Assist Participants with restorative and supportive care such as ambulating, positioning, transferring, filling oxygen, and providing other support as assigned or needed.
  • Follow clinical instructions as assigned.
  • Maintain accurate and current Participant care and service logs as required.
  • Assist the dietary staff with meal service and clean-up of dining areas.
  • Maintain personal care areas including but not limited to, coatroom, bath/shower rooms, personal care/linen supply areas, Participant rooms, and laundry areas.
  • Assist with stocking and maintaining personal care supplies for use in the Day Center.
  • Assist with laundry duties.
  • Assist with implementing recreation programs in a group setting and 1: 1 as needed including attending to needs of Participants during recreation activities and assisting Participants with recreation activities as appropriate.
  • Work collaboratively with other departments to ensure that the needs of Participants are consistently met and that a clean, sanitary, and safe environment is maintained.
  • Understand and adhere to PHH policies and procedures and PHH's Participant Rights Policy.
  • Protect privacy and maintain confidentiality of sensitive Participant information.
  • Attend staff and Participant care planning meetings as requested, and department in-services as required.
  • Report any event, accident, or unusual occurrence to the appropriate personnel.
  • Additional work functions and duties may be assigned.
