About The Position

The PACE Community Outreach Specialist is responsible for the recruitment and enrollment of PACE participants, community awareness efforts, and marketing of services to providers, referral sources, and the community. This role supports the health care team through community outreach, enrollment assistance, and education activities, providing information on health insurance, safety programs, and community resources. The specialist conducts intake interviews, attends community meetings, develops recommendations for healthcare system improvements, and organizes/participates in events and presentations. They may also provide training on health-related topics and health screenings. The position requires adherence to the Attendance and Absenteeism Policy and the ability to work at any FHCN location as needed.

Requirements

  • Proficiency in written and verbal communication, basic mathematics, computer applications, and technical systems.
  • Completion of an Associate’s Degree program with a recognized major and a minimum cumulative GPA of 2.5; OR a combination of relevant experience and completion of a high school diploma with a minimum cumulative GPA of 2.5, or General Educational Development (GED) with a minimum overall score of 162.5.
  • Healthcare-related knowledge frequently acquired through completion of a trade school, para-professional, or certificate type program.
  • A minimum of two years of experience conducting research; preparing written materials; and independently developing, coordinating, and executing promotional events, internal informational programs, and public information campaigns, including dissemination through publications, broadcasting, and/or public speaking (required).
  • Demonstrate knowledge of project management techniques, quality improvement principles, healthcare systems and managed care practices, and Medicare and Medi‑Cal benefits and regulatory requirements.
  • Work independently and as an influential member of a team.
  • Establish and maintain effective interpersonal relationships with all levels of staff, other programs, agencies, members, providers, and the public.
  • Communicate verbally and in writing effectively with individuals from varying cultures and backgrounds.
  • Prepare concise and clear reports.
  • Identify pertinent policy issues; develop strategies, solutions, and procedures.
  • Organize and track numerous and diverse activities, timelines, and issues.
  • Motivate and lead various participants at all levels in the organization.
  • Use computers effectively to process appropriate data and information and prepare reports.
  • Effectively utilize computer and appropriate software and interact with FHCN PACE Information Systems as needed.
  • Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from the date of hire to schedule for the next available CPR certification class provided by FHCN.
  • Job duties require compiling and analyzing information prepared in an effective written form, including correspondence, reports, articles, or other documentation.
  • Duties require preparation and execution of presentations to large groups.
  • The employee must move about the FHCN PACE center while performing the job duties and actively engage with the interdisciplinary team and participants.
  • Employees must be able to sit and work at the computer for extended periods.
  • Employees must use their hands and fingers, especially when typing on the computer and the mouse.
  • Employees must be able to talk and listen, particularly for phone communications.
  • The employee must frequently stand, walk, occasionally lift, and move up to 20 pounds.

Nice To Haves

  • A minimum of two years of experience in community outreach, educational activities, and customer service (preferred).

Responsibilities

  • Recommends, implements, and evaluates the overall marketing strategy for recruiting and retaining PACE participants and building and maintaining an optimal enrollee census.
  • Plans and coordinates advertising and promotional campaigns and strategies for the PACE Program.
  • Coordinates the production of printed publications and audio-visual productions, including developing design, layout, and text for informational materials.
  • Initiates, develops, and maintains public contacts for disseminating program information.
  • Drafts news releases that are submitted for higher-level approval through FHCN’s Marketing department.
  • Identifies potential public relations problems and advises program leadership and agency officials of appropriate preventative actions.
  • Coordinates ceremonies, press conferences, and other events or special projects.
  • Maintains administration and relevant reporting, tracking, and planning systems.
  • Plans and coordinates all marketing resources within an approved budget.
  • Supports the health care team through community outreach, enrollment assistance, and education activities, as assigned.
  • Provides information on health insurance, safety programs, and community resources.
  • Conducts intake interviews to determine eligibility for PACE.
  • Attends relevant community meetings as assigned.
  • Develops recommendations for improvements to the community healthcare system.
  • Organizes and participates in network-hosted events, community events, and presentations.
  • Provides training on health-related topics as assigned.
  • Provides health screenings at events.
  • Supports the implementation of PACE programs and grant-funded initiatives, as assigned.
  • Supports the implementation of program curriculum and/or objectives in community settings.
  • Supports supervisor in the collection of data and reporting.
  • Adheres to the Attendance and Absenteeism Policy.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Performs other duties as assigned.
