About The Position

Operation New Hope is a nonprofit organization dedicated to helping individuals impacted by the justice system successfully reintegrate into the community. We offer job training, placement, and comprehensive support services aimed at fostering self-sufficiency, preventing recidivism, and enhancing public safety. This specific role, the Community Outreach Specialist, is part of the Community Violence Intervention and Prevention Initiative (CVIPI) funded by a federal grant. The specialist will work collaboratively with the City of Jacksonville to implement community-driven strategies for improving safety, reducing violence, and providing essential support services such as workforce development, job placement, and mental health assistance to at-risk individuals and families.

Requirements

  • At least 3 years’ community outreach and engagement experience working with underserved populations.
  • At least 1 year of experience working with clients with mental health and/or substance abuse disorders.
  • Excellent verbal and written communication skills.
  • High integrity and behaves ethically, conducting oneself with the utmost honesty and transparency.
  • Proficiency in using computers, running reports, and learning new software.
  • Proficiency in Microsoft Office Suite; Word, Excel, PowerPoint, Outlook, Teams, and electronic client records.
  • Must be able to work Monday through Friday, with a flexible schedule that includes some evenings or weekend hours.
  • Must have a valid driver’s license, clean driving record and reliable transportation.
  • Must be able to clear a Level I Background check through the Department of Corrections.
  • Must be able to lift up to 15 lbs.

Nice To Haves

  • Bachelor’s degree preferred (Criminal Justice, Social Work, Sociology, or related fields)
  • 5 plus years of community outreach and engagement work in high crime neighborhoods.

Responsibilities

  • Manages a caseload of 25+ clients.
  • Completes Individualized Plans of Care, which include needs assessment.
  • Completes Risk Assessment with clients.
  • Facilitates employability assessments, which may include preparing a skill set profile, resume support, and one-on-one training/coaching.
  • Sets up interviews with potential employers and ensures clients are ready for job interviews.
  • Facilitates mock interviews with clients.
  • Connects clients to resources.
  • May provide bus passes, and other client resources as designated to clients seeking employment.
  • Enters and maintains accurate and timely touchpoints and case notes in ETO.
  • Coordinates with the Employment and Career Development team to schedule and assist with the completion of clients’ educational assessments.
  • Works with the Employment Team to complete client resumes.
  • Maintains an accurate list of services to support our clients and communicates updates to program staff.
  • Communicates professionally and effectively with staff, clients, family members, community partners and employers as needed.
  • Works with the Program Administrator and Community Engagement Lead/Finance to coordinate on-site community events.
  • Maintains documentation quality and integrity to meet program requirements, including weekly and monthly metrics.
  • Assists participants with securing vital documents, including, but not limited to birth certificates, Florida IDs, social security cards, and driver’s license replacement.
  • Maintains communication with employers and clients for 180 days following commencement of employment.
  • Participates in multidisciplinary staffing meetings.
  • Greets all visitors and clients entering the building with a positive, helpful attitude.
  • Other duties as assigned.
