About The Position

This is a career fair event for North County PACE, taking place on Thursday, June 4, 2026, from 12:30 PM to 4:00 PM at the Civic Center Library Community Room in Oceanside, CA. Attendees are required to complete an application prior to the event and bring an updated resume, a valid CPR/BLS card, and any relevant licenses/certifications. Interviews will be conducted for the following positions: PACE Personal Care Aide, PACE Certified Nursing Assistant, PACE Home Care Coordinator, and PACE Outreach Specialist. San Ysidro Health, the organizing entity, is a non-profit organization dedicated to improving the health and well-being of underserved and culturally diverse communities in San Diego County.

Requirements

  • Fluency in both English & Spanish
  • Updated Resume
  • Valid CPR/BLS Card
  • Any relevant licenses/certifications related to position (i.e. CNA, HHA, driver's license)
  • Six (6) months experience as a personal care aide, in a professional or personal care setting
  • High School Diploma, GED or six months experience as a personal care aide, in a professional or personal care setting.
  • High School Diploma, GED or one year experience as a CNA.
  • Nurse’s Assistant California State Certification
  • Current CPR Certification/Card
  • CNA certificate
  • Must obtain Home Health Aide certificate within 120 days of employment
  • High School Diploma or GED
  • At least 2 years’ experience working in a customer service role
  • CA valid driver’s license and reliable transportation

Responsibilities

  • Provide support in the delivery of personal care to participants.
  • Assist participants in need of companionship services to and from medical appointments.
  • Perform participant care activities such as showering, toileting, serving, feeding, and other Activities of Daily Living.
  • Assist with home delivery of meals and medical supplies.
  • Assist in other areas such as kitchen, primary care, and therapy.
  • Provide direct patient care services to participants under the direction and supervision of nursing staff.
  • Conduct home safety evaluations.
  • Coordinate and monitor IDT approved services, such as the delivery of meals, caregiver services, and DME.
  • Act as an advocate for participants' healthcare needs and preferences.
  • Coordinate and align unskilled home-based care with participants' goals and preferences.
  • Liaise with caregiver agencies, DME suppliers, etc., to coordinate schedules.
  • Monitor the delivery and quality of services/care provided.
  • Make recommendations for adjustments to care plans.
  • Conduct outreach in the community to raise awareness of and promote the PACE program.
  • Participate in events and fairs within the community.
  • Collaborate with Enrollment Specialists to increase census within the program.
