Overnight Private Duty Nurse (RN/LPN) - Temporary PDN Assignment with Long-Term Opportunity
About The Position
Home Care Network is seeking an exceptional Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide overnight private duty nursing services for a valued client in Kettering, Ohio. This opportunity was created due to a trusted member of our nursing team taking an extended medical leave. We are seeking two highly dependable nurses to provide consistent overnight coverage for a minimum of approximately three months while ensuring continuity of care for the client. This position is ideal for nurses seeking a stable, short-term overnight schedule, meaningful one-on-one patient care, and the opportunity to become part of a supportive, clinician-focused home health organization. This opportunity may also be an ideal pathway to future part-time or full-time opportunities with our agency, if desired and cases are available. A successful Private Duty Nurse at Home Care Network performs skilled nursing functions in accordance with federal, state, and local regulations, physician orders, and agency policies and procedures. The LPN or RN delivers client-centered care in the home environment while collaborating closely with the RN Case Manager and Clinical Leadership Team to ensure quality outcomes, excellent communication, and continuity of care. This role offers the unique opportunity to provide meaningful one-on-one nursing care while maintaining a consistent overnight schedule and a strong work-life balance.
Requirements
- Graduate of an accredited nursing program
- Active Ohio LPN or RN license in good standing
- Home health, private duty, long-term care, rehabilitation, hospital, or other relevant nursing experience
- Ability to work independently in a client's home environment
- Possess a strong track record of attendance, dependability, and employment retention with previous employers
- Strong clinical judgment and assessment skills
- Excellent written and verbal communication skills
- Current driver's license or state-issued identification
- Reliable transportation
- Active CPR and First Aid certification preferred (agency assistance available if needed)

Responsibilities
- Provides professional nursing care under the direction of a Registered Nurse.
- Communicates appropriate status/condition changes to the RN per agency guidelines.
- Provides skilled nursing visits for clients as directed by the RN Case Manager.
- Follows the care plan established by the Registered Nurse.
- Recognizes and reports changes in client condition to the Case Manager and physician, as directed.
- Communicates information on a timely basis, consistent with agency policy and urgency of the situation. Communication is verbal, as needed, and through complete and accurate documentation in the clinical record.
- Scope of clinical practice includes personal care services, assessment of findings, medication monitoring and administration, client teaching, emergency intervention, and nursing services within professional scope of practice and as delegated by the Case Manager.
- Individuals with the license designation of "LPN M-IV" may perform venipuncture for the purpose of obtaining blood samples for laboratory analysis.
- Promotes personal safety and a safe environment for clients and coworkers.
- Demonstrates knowledge of safety/infection control practices by compliance with policies and procedures.
- Recognizes and responds appropriately to potentially unsafe situations.
- Demonstrates safe practice in the use of equipment.
- Makes recommendations on improvement of safety.
- Performs job in compliance with agency policies and procedures and professional and community standards.
- Accepts responsibility for professional nursing role.
- Follows applicable state and federal laws and agency standards.
- Attends all meetings as required.
- Maintains confidentiality in all aspects of the job. Refrains from discussing client or staff identities, conditions, or circumstances in public or with others who do not have a legitimate interest in the person.
- Participates in the development, implementation, and evaluation of the Agency Quality Improvement Program and pertinent activities.
- Performs other related duties and responsibilities as deemed necessary.
