Requirements

  • High school diploma or equivalent; must be at least 21 years of age.
  • Active LPN or RN license in the State of Utah.
  • Valid Utah driver’s license with a clean driving record.
  • Ability to pass a Utah State background check.
  • Strong communication skills and the ability to work collaboratively in a team setting.
  • Proficiency in medical terminology, clinical charting, and medication administration.
  • Skilled in physical health assessments and standard nursing tools.
  • Comfortable with word processing software and basic network navigation.
  • Willingness to receive professional feedback and participate in ongoing training.
  • CAQH profile and NPI number required for credentialing.

Responsibilities

  • Provide comprehensive nursing care, including routine health needs and medical emergencies.
  • Administer and document medications accurately, including controlled substances.
  • Conduct timely health assessments and monitor vital signs, weight, mood, and behavior.
  • Coordinate lab work and accompany residents to medical appointments.
  • Communicate relevant medical information to parents, the Nursing Director, and Treatment Team.
  • Collaborate with therapists and psychiatric providers to support treatment plans.
  • Participate in an on-call rotation ensuring 24/7 medical support.
  • Maintain HIPAA compliance and accurate clinical documentation.
