About The Position

Journey Home is a program for 6 adult clients that have been given a Not Criminally Responsible (NCR) Disposition. Staff are onsite, 24 hours a day, 7 days per week and provide supervision and support in a safe, stable environment to help residents gradually readjust to the community after living in a secure hospital setting. This Program is delivered in partnership with the FACS NCR Treatment Team. Overnight Caseworkers are a part of a full-time team responsible for the day-to-day operation of the Home, teaching basic living skills, creating client SMART goals, and facilitating community activities and outings.

Requirements

  • Degree or Diploma in human services field with related experience
  • Clear Criminal Record Check, and valid First Aid/CPR
  • Able to work consistent awake overnight shifts between the hours of 11:00 pm and 7:00 am.
  • Flexibility to work an on-call rotation is required
  • Strong communication and pro-social relationship building skills
  • Demonstrated ability to communicate effectively with a mental health population
  • Ability to work independently and as part of a multidisciplinary treatment team
  • This is an awake overnight position that requires the selected candidate to work overnight hours between 11:00 pm – 7:00 am, Monday to Friday.
  • The position requires frequent standing, walking up and down stairs, and occasional lifting.
  • 1 – 3 years of related experience

Responsibilities

  • Completion of accurate written daily documentation (log book entries, contact notes, incident reports, medication book entries)
  • Ongoing monitoring of client files to ensure they are kept complete and up to date (monthly file reviews, goal setting, discharge summary, weekly meetings, etc.)
  • Maintain accurate and up-to-date data records, including data collection, data entry, and completion of program assessments/surveys
  • Role model and assist clients with self-advocacy
  • Adapt services to meet the needs of diverse populations as per unit policy
  • Deliver services using a client centered, strength-based approach
  • Liaise with the FACS Treatment Team and various other treatment professionals
  • Attend client case conferences, unit and agency meetings, and staff training
  • Ensure services are being delivered in accordance with Agency Policies and Procedures
  • Contribute to a positive culture by assisting and supporting colleagues as needed, and encourage and teach others about the unit’s operations
