Outreach Worker

Urban Peak Denver•Denver, CO

About The Position

The Outreach Worker, under the supervision of the Outreach Program Manager, connects with youth experiencing homelessness to build healthy relationships and support their progress toward self-sufficiency. The role represents Urban Peak in the community, informs youth and community members about available services, and helps homeless and runaway youth improve their health, safety, and well-being and exit street life. Work includes street and shelter outreach, coverage of shifts at the Mothership, case management, referrals, and facilitating activities. Interactions and environments must be trauma-informed and support the physical and emotional safety of youth and staff. The position also serves people in large Denver encampments in coordination with partner-agency case managers, outreach workers, peer support specialists, and street nurses. Services may include crisis intervention, basic-needs supplies, access to shelter and other housing options, medical and behavioral health care, COVID-related care, transportation, advocacy, and service linkage. The worker builds relationships with city departments and other entities to support people in encampments, with particular focus on youth under age 25 and connecting them to longer-term case management. The position performs outreach, case management and referrals, milieu supervision, documentation, intakes, and assessments. Duties are performed individually and in coordination with supervisors, colleagues, and partner agencies; additional work may be assigned. The schedule is Monday through Friday, 8:00 a.m. to 4:00 p.m., with three holidays and three on-call shifts per year.
