About The Position

The Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton is seeking a caring and compassionate individual to join our team as an Outreach Support and Resource Coordinator. The Outreach Support and Resource Coordinator plays a key role in supporting individuals connected to the organization by providing ongoing support, program coordination, and connection to resources. In this role, you'll be a welcoming point of connection for care partners and individuals living with dementia to help ensure that no family feels alone on their dementia journey. Requirements

  • Diploma or degree in Social Services, Social Work, Psychology, Gerontology, Human Services, Nursing, or related field.
  • 2-3 years' experience working in healthcare, social services, community services.
  • Experience working with older adults, people living with dementia and their care partners
  • Experience in assessment, care planning, and service coordination
  • Travel throughout the four-county region
  • Valid G Class driver’s license and access to a reliable, insured vehicle
  • Vulnerable Sector Police Check (within the last 2 months)

Responsibilities

  • Build and maintain supportive relationships with care partners and individuals living with dementia to identify evolving needs and connect families to appropriate supports.
  • Provide information, guidance, and practical support to help people navigate available programs and community services.
  • Coordinate specialized initiatives including Project Lifesaver® and Finding Your Way® Ontario.
  • Coordinate volunteers involved in counselling programs.
  • Provide back-up coverage for care partner support groups and social programs.
  • Provide administrative support for counselling program operations and special projects.
