About The Position

The Outreach Specialist will be responsible for community and street outreach to identify veterans and their families at risk of becoming homeless, the most vulnerable homeless individuals, and chronically homeless veterans in Fresno and Madera Counties. This role also involves conducting outreach for Point in Time Surveys, Vulnerability Surveys, and Homeless Encampment closures. The specialist will be an integral part of an outreach team utilizing the Housing First Model and a comprehensive centralized assessment system.

Requirements

  • Two (2) years of experience providing direct outreach services in an intensive setting.
  • Ability to work flexible schedule (schedule may include weekends and evening hours).
  • Ability to pass a criminal history background screening.
  • Valid drivers license and ability to obtain and maintain WestCare's vehicle insurance.
  • CPR Certification.
  • First Aid Certification.
  • Annual Tuberculosis test.
  • GED or High School Diploma is required.
  • Ability to obtain and maintain Fresno County Credentialing.
  • Ability to obtain and maintain clearance from California Department of Corrections and Rehabilitation (CDCR).
  • Full vaccination against COVID-19 effective 12/08/2021, as mandated by Presidential Order for all employees of WestCare Foundation, Inc. and its affiliates.
  • For employees working at Residential, Outpatient, Sober Living, providing Mental Health Services, Administrative Staff or Transitional Living Facilities, full vaccination and a booster are required as mandated by the State of California. Employees with approved Medical or Religious Exemptions will be required to test for the virus weekly.

Nice To Haves

  • Veteran and/or military experience preferred.
  • Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related field is preferred.

Responsibilities

  • Serve as an active part of the San Joaquin Valley Veterans (SJVV) outreach team and carry out all required responsibilities as needed.
  • Provide screening, assessment, and brief interventions to the homeless and those at risk of becoming homeless.
  • May be required to provide referrals to community partners, case management services, education, and appropriate community resources.
  • Participation in facilitating ancillary services, i.e., continuing care, networking with community agencies, participating in Homeless Connect events and developing and updating referral and resource information, etc.
  • Participation in case staffing, team meetings and Fresno Madera Continuum of Care Outreach Committee meetings as required.
  • Coordinate and partner with community resources to promote the wellbeing of the homeless and veterans (i.e., WestCare, Fresno Housing Authority, Fresno County Department of Social Services, VA, Vet Center, FMCoC).
  • Develop and maintain working knowledge of available community resources and act as liaison.
  • Conduct outreach activities in the Fresno Madera urban and rural communities including street outreach.
  • Attend scheduled meetings within the community.
  • Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform any other duties as assigned.
