About The Position

The HEART of Communities (HEART) Outreach Specialist is the first point of contact for prospective program participants. The role engages eligible patients, provides program information, supports enrollment, and maintains accurate documentation to coordinate effectively with Community Wellness Advocates (CWAs) and clinical teams. In partnership with the Operations Manager, the Outreach Specialist supports the development and improvement of outreach workflows, enrollment goals, and special projects that advance HEART’s growth and strategic objectives. This is a full-time position in the MGB Diabetes Initiative department. Compensation is listed as $18.27–$25.72, based on Boston-area data and subject to modification by geographic location.

Requirements

High school diploma and a minimum of 1 year of customer service experience, or an equivalent combination of education and experience.
Strong oral and written communication, interpersonal, and organizational skills.
Proficiency with software applications, including Microsoft Office and electronic health record systems such as Epic.

Responsibilities

Conduct outreach calls and chart reviews for new eligible HEART patients.
Schedule intake appointments for patients who agree to participate.
Assist with rescheduling patients who no-show or cancel their initial program appointment.
Document baseline clinical data in Epic systems and maintain the Epic program database for new patients.
Assist with outreach data management, developing efficient workflows, and documenting outreach workflows and standard operating procedures.
Assist the HEART program with special projects assigned by the Operations Manager.
