About The Position

AmerihealthCaritasDC maintains a list of at-risk members who admitted to local emergency rooms for mental health emergencies. The PEES contractor must locate these individuals and assist in conducting a behavioral health screening. This position consists of three main phases: engagement, investigation, and referral/completion. Engagement: The PEES will be assigned members based on criteria determined by AmerihealthCaritasDC. PEES will have access to information about the member through the CRISP and Credible systems, when available. Investigation: PEES will conduct at least three substantial outreach attempts as defined by agency established standards. Additional attempts may be conducted. This includes in person outreach attempts. All outreach attempts will be documented in the member's Credible file. Documentation must be descriptive. Investigation activities are inclusive of the Engagement rate. Referral/Completion: Once the member has been located, PEES documents the member's residence or other location, as well as any other information that will aid in continuing care and follow-up. PEES contacts the licensed clinician to conduct a behavioral health screening. When the assessment is completed and the PEES staff member has completed required documentation in Credible, the contractor is eligible for reimbursement.

Requirements

  • High school diploma required.
  • Strong engagement skills.
  • Comfortable in community settings including homes, shelters, and residential programs.
  • Works effectively in an independent setting.
  • Strong time management and organization skills.
  • Able to travel independently in the community to various locations in DC on a daily basis.
  • Strong written skills.
  • Experience in EMR a plus.
  • Will utilize electronic healthcare systems to review consumer history and needs.
  • Agree to all terms and conditions associated with access to confidential and protected health information (PHI).
  • Exercise discretion when opening and/or downloading PHI to ensure its confidentiality and safekeeping.
  • Return all documentation, records, or other data belonging to the McClendon Center at the conclusion of the contract.
  • Operate a fully insured vehicle at all times.
  • Report any moving violations or automobile accidents immediately.
  • Report any lapse of automobile insurance immediately.
