About The Position

This role involves contacting and scheduling patient/client appointments for consultations, meetings with community agencies, and diagnostic tests. The associate will also be responsible for setting up and maintaining files and records, and collecting and organizing data for statistical reports.

Requirements

High School Diploma or equivalent required.
0-1 years of relevant experience, required.
Ability to work effectively with diverse populations and provide services in a respectful, nonjudgmental, and stigma-free manner.

Nice To Haves

Associate's degree in Public Health, Health Education, Human Services, Social Work, Community Health, or a related field preferred.
Experience working in HIV/STI prevention, community outreach, public health, sexual health, infectious diseases, or a related field strongly preferred.

Responsibilities

Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests.
Collects and organizes data required for statistical reports and completes reports, as required.
Attends and participates in meetings as appropriate.
Sets-up files and maintains records.
Provides incoming and in-process orientation.
Based on position, may be required to provide bi-lingual translation and/or guidance.
Coordinate community outreach activities designed to increase awareness of HIV, HCV, Syphilis, PrEP, PEP, sexual health, and available prevention services.
Develop and maintain relationships with community-based organizations, shelters, healthcare facilities, schools/colleges when appropriate, social service agencies, and other community partners.
Identify opportunities for testing events and outreach activities in communities with increased need.
Assist with planning health fairs, mobile testing events, educational presentations, and targeted outreach initiatives.
Support culturally responsive and stigma-free approaches to sexual health education and testing.
Performs related duties as required.
