About The Position

The Crisis Center of Tampa Bay brings help, hope and healing to people facing serious life challenges or trauma resulting from sexual assault or abuse, domestic violence, financial distress, substance abuse, medical emergency, suicidal thoughts, emotional or situational problems. The Outreach & Recruitment Coordinator conducts community outreach to inform providers about services offered, eligibility criteria, how to refer, and explores prospective participants. Outreach activities include but are not limited to small group meetings, presentations, tabling events, and regularly scheduled email and phone contact with community partners. The Outreach and Recruitment Coordinator reports to the Healthy Minds Program Manager, and in their absence, the Clinical Director.

Requirements

  • Master’s Degree in a social service field preferred
  • Bachelor’s Degree in a social service field
  • Minimum of 3 years of experience providing behavioral health services and community outreach activities
  • Bi-Lingual (Spanish/English) preferred
  • Strong analytical ability and proficiency with applicable technologies and software utilization.
  • Effective presentation, written and verbal communications skills.
  • Knowledge of community resources.
  • Ability to collect and evaluate data.
  • Ability to prepare reports relating to clients.
  • Ability to work independently.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to problem solve.
  • Ability to organize and prioritize work.
  • Skill in operating Windows based computer operating programs.
  • Knowledge of and compliance with HIPAA regulations.

Responsibilities

  • Responds to clients using agency, contract and program policies and procedures.
  • Advocates for services and resources for families
  • Assist and participate in research activities required for grant sourcing and proposals.
  • Conduct outreach to community partners to make them aware of the Healthy Minds program.
  • Involves staff and other stakeholders in the performance quality improvement (PQI) process and uses data to improve services and outcomes.
  • Develop a knowledge base of prospective referral sources, and build and maintain relationships with those identified
  • Conduct outreach activities with community providers via email, phone, and in person to provide information about eligibility criteria, services provided, and how to refer to the Healthy Minds program and other S4KF programs
  • Track Outreach and Recruitment as well as Information and Referral activities and efforts to be submitted monthly.
  • Successfully complete training programs required for program adherence and participant support.
  • Receive and enter referrals made via phone, fax, or email
  • Contact prospective participants and/or their guardian to complete initial screening to determine eligibility and, if eligible, offer and explain Healthy Minds services
  • Correspond with referral sources to gather information necessary to determine eligibility
  • Confer with Program Manager to determine appropriateness of referrals to the Healthy Minds Program
  • Manage client data in the Electronic Health Record to include successful and attempted contacts
  • Assess prospective participant and family needs and take proactive steps to provide information for applicable community support when needed
  • Meet with Program Manager for regularly scheduled individual supervision
  • Participate in weekly Case Staffing meetings with the Healthy Minds team
  • Coordinate with the Program Manager to assign new participants to Healthy Minds Primary Clinicians
  • Develop a comprehensive understanding of all internal and community programs and services available to clients, and maintain an awareness of changes in program/service offerings and requirements
  • Recognize and act on reportable events and determine appropriate course of action in emergency or high-risk situations under the coaching and guidance of the Program Manager
  • Performs other such duties as may be assigned by the Healthy Minds Program Manager or Clinical Director.
