About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona, Oklahoma, and the District of Columbia. Founded in 1982, CBI's mission is “maintaining the dignity of human life” for over 40 years, providing residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. The District of Columbia Stabilization Center (DCSC), operated by CBI, offers immediate, 24 hours/day, 365 days/year, no cost, low barrier access to Crisis SUD services for individuals 18 years and older, regardless of gender, insurance, and citizenship status. The DCSC provides a person-centered and recovery-oriented alternative to emergency departments or law enforcement interactions, ensuring integrated and comprehensive care in a supportive clinical and recovery-oriented environment. CBI Outreach Navigators work closely with Peer Operated Center program participants daily to establish short-term and long-term goals, focusing on employment stabilization to achieve successful housing and income stability. Navigators maintain continuing contact with participants to monitor their transition and assist them with accessing needed benefits and services.

Requirements

  • Highschool diploma or GED is required
  • Minimum of six months of recovery from substance use and/or mental health disorders required

Nice To Haves

  • Associates Degree (or higher) in a field related to Behavioral Health is preferred
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required

Responsibilities

  • Work closely with Peer Operated Center program participants on a daily basis to establish both short-term and long-term goals with a focus on employment stabilization goals to achieve successful housing and income stability
  • Provide continuing contact with participants to monitor their transition from non-POC engagement to POC enrollment
  • Assist participants with accessing needed benefits and services, such as Medicaid, social security, disability, behavioral and mental health treatment, legal assistance, medical care, education, employment-related services, identification and transportation
