About The Position

The Outreach and Enrollment Specialist plays a vital role in expanding the visibility and impact of the health center across all service areas. This individual will foster community relationships, promote clinic services, and implement outreach strategies that increase patient engagement, enrollment, and utilization of health services. This position supports strategic goals through onsite and mobile outreach efforts and works closely with internal teams to drive participation across all service lines, including primary care, dental, behavioral health, senior services, OB/GYN, and urgent care.

Requirements

  • Two years’ experience working in a community social service or healthcare environment.
  • Bi-Lingual in Spanish and English required.
  • Must have excellent reading and comprehension skills.
  • Must be able to work efficiently in a very fast-paced environment.
  • Must adhere to the agency’s non-discrimination policies.
  • Experience working with low-income and predominantly Latino populations.
  • Experience working with ethnically and culturally diverse communities, and low-income populations.
  • Ability to effectively interact with co-workers and patients with diverse ethnic backgrounds, religious views, political affiliations, cultural backgrounds, lifestyles, and sexual orientations and treat individuals with respect and dignity.

Nice To Haves

  • Current HIV/STI Counseling Certificate preferred, as well as current training on family planning practices and methods.
  • Covered CA Enrollment Assister Certification strongly preferred.

Responsibilities

  • Attend, organize, and represent Via Care at community events, health fairs, and school or senior center functions.
  • Present clinic services to a wide range of stakeholders, including schools, local businesses, nonprofits, faith-based groups, and housing authorities.
  • Build and maintain partnerships with local organizations, agencies, and advocacy groups to generate referrals and collaborations.
  • Identify new partnership opportunities to align community needs with clinic service lines.
  • Coordinate closely with enrollment, mobile units, and clinic operations to ensure smooth onboarding and care continuity.
  • Track and report weekly outreach outcomes, including patient conversions, engagement rates, and partner activities, enrollment appointments.
  • Ensure consistent messaging and branding across outreach efforts, distribute materials.
  • Support outreach across all service areas of our sites throughout East Los Angeles.
