About The Position

The Outreach Case Manager would provide a holistic approach to case management services and providing outreach programs to participants who are returning from incarceration and/or at risk of becoming incarcerated. This position will require effective interaction and coordination with participants, staff members, volunteers, and other agency representatives to facilitate client success.

Requirements

  • High School Diploma or equivalent required.
  • At least one-year of experience providing case management services.
  • Working knowledge with local Human Services.
  • Ability to pass pre-hire screenings such as background or drug screenings.
  • Valid drivers license and access to a vehicle on a continual basis.

Responsibilities

  • Engage in regular outreach to returning citizens with screenings and intakes.
  • Ability to interact with participants that are in situations such as low-income, post-addiction, and incarceration.
  • Knowledge of intake and interview techniques with a holistic approach.
  • Provide case management with ability to assess situations and work independently to develop successful action plans with goals and objectives addressing the individuals current needs while preserving the participants dignity.
  • Maintain contact with participant and ensure follow through with the goals, objections, and expectations created.
  • Scheduling meeting times with participant by being creative and flexible to their needs. Traveling is required.
  • Coordinate with local community services to provide available programs for participants.
  • Develop a relationship with local law enforcement, detention centers, and community corrections.
  • Ability to work as part of a case management team and build effective working relationships with co-workers and other agencies/organizations.
  • Maintain accurate case records, reports, and statistical data.
  • Excellent interpersonal, verbal, and written communication skills.
  • Working knowledge of standard office procedures and report preparation.
  • Proven ability to maintain confidentiality within and outside of agency.
