The Bilingual Outreach Case Manager works collaboratively with the Enabling Services Department, medical providers, and primary care teams to connect community members with services offered through Keystone Health. This role focuses on community outreach and education to increase awareness of Keystone services and appointment availability. The Outreach Case Manager provides short-term care coordination, social support, informal counseling, and culturally responsive education to targeted populations, with the goal of improving health outcomes and overall well-being. The ideal candidate has strong connections within the community and is knowledgeable about available local resources. They are self-motivated, able to work independently, and dedicated to helping community members access services aligned with their needs.
