About The Position

Provide outreach and intervention services to homeless customers in Bell, Harlan, Knox, Jackson, Laurel, Rockcastle, and Whitley counties seeking shelter and supportive services, work to identify existing services available to clients and work with others in developing new services to meet the needs of the clients. Areas of focus include intake, orientation and assessment, family services, children’s services, recovery program services, veteran’s services, and self-sufficiency services; oversees and acts as a “troubleshooter” for the daily operations of work station as it relates to the efficient delivery of services to clients Makes clear and direct referrals to each client seeking services to address the specific needs identified in assessment and self-sufficiency planning sessions Process clients through the rapid re-housing programs, including processing rental and utility payments monthly Maintains case management records (both written and in the Homeless Management Information System (HMIS), progress notes, and service related information on individual cases; enters data into statewide homeless management system and others as deemed necessary; prepares other related reports as required Work cooperatively with other outreach staff and case managers, program directors, and partnering agencies in coordinating services to prevent the duplication of services; and, to ensure the client is being served in a cost efficient and effective manner Assists each client in accessing and receiving mainstream services and to evaluate those experiences as they relate to their present condition To establish and maintain contact with homeless customers during the process of locating emergency housing or housing that is transitional in nature, and up to a period of six months after they leave a shelter or obtain more permanent housing; will encourage former homeless and community volunteers to become mentors to help others become self-sufficient As an advocate, seeks to help homeless customers identify the personal barriers that contribute to their inability to meet goals and assist them in developing personal goals and an action plans leading to self-sufficiency Coordinates with all staff, community agencies and homeless programs, and volunteers in determining appropriate education and self-help activities for homeless customers, such as life management skills, parenting education, job hunting, adult education, vocational training, child care, etc Cooperatively work with shelters, housing programs, and other resources to provide ongoing educational and self-sufficiency activities that will provide homeless customers the opportunity to increase their skills to achieve their goals Attends all staff meetings, training and other meetings deemed necessary by to gain knowledge of services for the clients and for the effective delivery of services and/or required by the agency Practices safety in all aspects of the job, maintains confidentiality and security of homeless customers, and adheres to program standards regarding communicable diseases and blood born pathogens Performs other duties as assigned as it relates to the delivery of services made available by the designated programs

Requirements

  • High School graduate/equivalent
  • willing to work flexible schedule, including evenings and weekends

Nice To Haves

  • sociology, social work, psychology, or similar field degree preferred
  • three to five years work experience in human services preferred
  • working knowledge of computer programs
  • supervisory experience helpful
