As a Street Outreach Case Manager, your mission is to help individuals facing mental health or substance use challenges secure stable housing and rebuild their lives. Every day, you’ll be out in the community—on the streets, in shelters, libraries, emergency rooms, and other public spaces—meeting people where they are. You’ll work closely with service providers to break down barriers, open doors to vital resources, and help individuals take the next step toward stability and independence. Community Housing Advocates (CHA) a parent company of Mercy Housing & Shelter and My Sister’s Place, two agencies who in 2018 united forces based on a common mission- combating homelessness and providing support to individuals and families in need. We provide safe housing, life-changing services that include medical care, management of financial affairs, short- and long-term housing and all the necessary tools to reclaim their independence.
