Jesuit Social Services is seeking two experienced Case Managers for the ReConnect program, which provides transitional support to men and gender diverse individuals exiting adult prisons in Victoria, focusing on the North and West metropolitan regions of Melbourne. The program aims to facilitate community reintegration and reduce recidivism by addressing critical domains such as housing, mental health, alcohol and other drugs (AoD), employment, education and training, independent living skills, and family and community connectedness. These roles involve managing caseloads of participants with multiple complex needs, including those assessed as high risk of recidivism, high reintegration support needs, and those who have engaged in high profile, violent, and sexual offending. The Case Managers will be expected to work directly with this cohort, facilitating discussions on sensitive topics like sexual offending and working with men who use violence. The program is dynamic, fast-paced, and requires adaptability due to high administrative demands. It is funded by the Department of Justice and Community Safety (DJCS) and is part of the Corrections Victoria Reintegration Pathway (CVRP), supporting individuals to reintegrate into the community upon release. Key objectives include implementing individualized transitional plans, providing assertive outreach and practical assistance, and helping participants address the underlying causes of their offending to achieve positive outcomes.
