About The Position

This is a community-based position providing direct, face-to-face services to individuals who are homeless and travel throughout Sacramento. This position is responsible for delivering mental health, substance abuse, and supportive services consistent with the principles of the Psychosocial Rehabilitation & Strengths Model to an adult population experiencing homelessness and struggle with mental health. This position provides recovery & strengths-based community outreach, case management, housing navigation, social rehabilitation, skills building, benefits & resource support, group facilitation and crisis intervention.

Requirements

  • High School Diploma or GED (required)
  • Four (4) years of experience in a Mental Health or Drug and Alcohol Rehabilitation related field providing Mental Health or Drug and Rehabilitation services to individuals experiencing mental health challenges and/or homelessness.
  • Two (2) years direct care experience in a Mental Health or Drug and Alcohol Rehabilitation related field providing Mental Health or Drug and Rehabilitation services to homeless individuals; AND two (2) years of education/training in a Mental Health or Drug and Alcohol Rehabilitation related subject area such as social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling (CAAR, CADAC, etc).
  • Bachelor’s degree in a Mental Health or Drug and Alcohol Rehabilitation related field.

Responsibilities

  • Responsible for community outreach face to face screening for eligibility and enrollment into the program.
  • Acts as liaison between client, identified support persons, and community service providers.
  • Assists with coordination and delivery of preventive health care programs and benefits supportive services.
  • Provide continuous outreach engagement and crisis intervention to clients in the field/ onsite.
  • Conducts intake and risk assessments with clients and support persons to obtain information regarding Psycho/social/medical history.
  • Assess ongoing service necessity and creating a recovery oriented strengths based treatment plan with the client.
  • Provides assistance to Service Coordinators/Clinicians/Peers with the development of assessments, service plans, case management, housing support and crisis intervention.
  • Is effective at prioritizing/adjusting to the needs of the client and/or clinic as mean to create quality services for the client and meeting clinic goals.
  • Distributes educational materials, donated items and outreach kits/supplies to homeless individuals.
  • Contacts & provides individuals in various community settings about community programs in Sacramento County including education/presentation about CORE Mariposa services.
  • Provides community stakeholders with educational information concerning behavioral health/social service programs in Sacramento County.
  • Provides transportation for clients to attend mental health/medical/ benefits appointments.
  • Support clients/guests in the Community Wellness Center by obtaining supportive services identified by the guest/client; Complete and support with HMIS assessments and service data entry for clients and guests experiencing homeless, chronically homeless, or at risk homelessness.
  • Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by supervisor.
  • Responsible for maintaining all charting records and documentation in accordance with El Hogar and Sacramento County regulatory policies & procedures.
  • Responsible for producing required number of regulatory units of service per month (70% of staff’s time must be providing face-to-face or other eligible community-based support with/for client).
